Sprout Health California is committed to providing evidence-based, clinically-proven, and remarkably effective treatment programs to our clients.
These include programs that address substance use disorders, mental health issues, and co-occurring conditions. We accept referrals from other healthcare organizations and providers and help to facilitate a seamless transition to our facility. This allows for a faster and straightforward communication of necessary documentation and medical information regarding prospective clients. In doing so, our Intake and Referral Coordinators are better equipped to formulate a unique and truly individualized treatment plan for the potential client.
As such, we have devised numerous resources designed to aid both clients and referrers transition into our healthcare system as seamless as possible. These resources are available throughout the admission, treatment, and discharge phases. Our goal at Sprout Health California is to provide unmatched customer service to consolidate what would normally be a challenge the currently accepted health care referral process.
Sprout Health California strives to eradicate the challenges that accompany the majority of common patient admissions systems which are often slowed by regulatory processes. We understand this and provide all referring individuals, professional, and organizations with accurate information and guidance pertaining to our treatment centers and therapeutic treatment programs.
If the client chooses to receive treatment in one of our facilities, Sprout Health California will collaborate with the referring health care professional to gather as much of the necessary information pertaining to the client, such as their medical records. This information is required to in order to develop a treatment plan catered indicatively to the client, their needs, and health status. We take great care to streamline the process, while still maintaining legal and ethical integrity. This includes the specific clauses of confidentiality in relation to the patient and the Health Insurance Portability and Accountability Act.
Client Treatment Progress Update
To further ensure adherence to legal and ethical guidelines, specifically HIPAA and other laws regarding consent, our Intake Coordinator will request the client to authorize the disclosure of their personal health information. While Sprout Health California respects the referral from the contributing medical professional and the possibility they may want to be updated on the client’s progress, there may be particular pieces of information that cannot be disclosed. The exception being written consent of the client.
If the client gives permission to share information with his referring medical professional, our referral coordinator will act as a liaison and keep the referrer up-to-date regarding the patient’s treatment and discharge strategy. This is critical for the healthcare professionals and referrers who will continue to be an essential part of the client’s care and recovery once they leave the program. The authorized professionals will be updated within 72 hours after admission to the facility to discuss a strategy for future updates.
Authorized professionals will also be privy to the client’s discharge plans and the referral coordinator will communicate these arrangements to the referrer. Sprout Health California strives to foster successful treatment and long-term recovery and as such, we attempt to ensure the client receives continued care. The goal of discharge planning is to ready the client and ongoing health professionals for their release from the treatment program. Our hope is that our client’s will leave equipped with the tools needed for sobriety and mental wellness and the continuation of care will bolster their likelihood for success. To that end, the referrer needs to be able to prioritize the continued support the client may need such as fortifying treatment tools to further facilitate the client’s recovery and mental wellness.
Sprout Health California strongly believes substance misuse, addiction, and mental illness negatively affect society and its solution requires societal cooperation. By streamlining and eradicating the plethora of obstacles that arise during admission or referral process, Sprout Health California is able to better serve the individuals seeking mental health or substance use treatment. We also appreciate the backing of referring health care professions. Without them Sprout Health California could not provide our level of addiction treatment or mental health therapy to all of those we do.
